De ce fait, l&rsquo;incapacit\u00e9 \u00e0 trouver un professionnel de sant\u00e9 entrave la sant\u00e9 de la population. C&rsquo;est pourquoi, la CPTS DPV souhaite aider les personnes en recherche de prise en charge continue et\/ou urgente au travers de 2 dispositifs.<\/strong><\/p>\n                    <p><strong style=\"font-weight: 700;\">Le premier est focalis\u00e9 sur la recherche d&rsquo;un m\u00e9decin traitant et le second sur la recherche d&rsquo;une consultation en soins non programm\u00e9s <em>(en moins de 48h)<\/em>.<\/strong><\/p>\n                    <ul>\n                      <li><strong style=\"font-weight: 700;\"><em>M\u00e9decin traitant<\/em> : Gr\u00e2ce \u00e0 l&rsquo;implication des m\u00e9decins adh\u00e9rents pr\u00eats \u00e0 accepter de nouveaux patients, la CPTS peut proposer, sur \u00e9tude du formulaire de demande, une orientation individuelle vers un m\u00e9decin.<\/strong><\/li>\n                      <li><strong style=\"font-weight: 700;\"><em>Soins Non Programm\u00e9s (SNP)<\/em> : Afin d&rsquo;\u00e9viter l&rsquo;engorgement aux urgences et\/ou le renoncement aux soins, la CPTS participe \u00e0 la mise en place du dispositif SAS <\/strong>\n                        <a href=\"https:\/\/www.bing.com\/search?q=dispositif%2Bd'acc%C3%A8s%2Baux%2Bsoins&amp;qs=n&amp;form=QBRE&amp;sp=-1&amp;pq=dispositif%2Bd'acc%C3%A8s%2Baux%2Bsoi&amp;sc=1-26&amp;sk=&amp;cvid=78236D7E08364874B06ECF28C75B6989&amp;ghsh=0&amp;ghacc=0&amp;ghpl=\"\n                          style=\"font-weight: 700;\" class=\"customize-unpreviewable\"><strong>(Service d&rsquo;Acc\u00e8s aux Soins)<\/strong><\/a><strong style=\"font-weight: 700;\"> sur notre d\u00e9partement. Pour cela, les m\u00e9decins adh\u00e9rents du territoire mettent \u00e0 dispositions quelques cr\u00e9neaux de rendez-vous de SNP . La CPTS, en lien avec le SAMU, g\u00e8re ces cr\u00e9neaux de Soins Non Programm\u00e9s. <\/strong><\/li>\n                    <\/ul>\n                    <p>\n                      <a
